All products are “owned” by the seller until paid in full by the purchaser – at which time title passes to the purchaser. MedRequisite.com is not responsible for warranties offered by manufacturers on any products sold through this website, or for the suitability or capability of any manufacturer’s stated use of their product. Any purchased items that are for resale or export to a foreign country will require Federal (U.S.) export documentation before shipping. Moreover, any purchases on this website bound for a foreign country may alter or void applicable warranties altogether.
Terms & Policies
MedRequisite.com retains the right to make adjustments on prices and products as a result of manufacturer product availability, changing market conditions or misrepresentations in advertisements by the manufacturer or distributor. A product image on this website may not necessarily represent the product the purchaser receives. Alterations, color variations, or unavailable updated images at the time of purchase, may exist.
It is important to keep all packing documentation, including manufacturer’s warranty information, product manual(s) and shipping details, in the event your product requires service, replacement or return.
Prior to any product returns, purchaser must first obtain an RMA (return merchandise authorization) from MedRequisite.com. See Return and Exchange Policy Section.
All defective or unusable products will be accepted at MedRequisite.com’s discretion for either exchange or credit within 30 days of receipt of purchase. Any request for return after 30 days will be subject to the manufacturer’s warranty only, if applicable.
A product that is non-defective in resalable condition may be returned within 30 days of receipt of purchase.
MedRequisite.com reserves the right to provide an RMA number for returns that exceed 30 days of receipt of purchase. Those returns accepted after 30 days would be reflected as an account credit to be utilized for future purchases only.
Hygiene, wound care, and bathroom safety products may not be returned due to company hygiene requirements.

Any information, ideas or unsolicited suggestions that are conveyed or communicated to MedRequisite.com via the MedRequisite.com website, email, chat or phone conversation, will become and remain exclusively, the property of MedRequisite.com.
If your personal information is established with MedRequisite.com for any purpose, it is the customer’s responsibility to notify MedRequisite.com of any changes to this information when new orders are placed or existing order are in progress. Failure to notify MedRequisite.com of any updated information may result in additional shipping charges or restocking fees to the customer due to outdated information on file.
MedRequisite.com reserves the right to make changes to the Privacy Policies at any time to ensure protection of customers private health information.
MedRequisite.com complies with the Health Insurance and Portability Act of 1996.
Return and Exchange Policy
Our company goal is for all customers to be absolutely satisfied with their purchase. If a return or exchange is required, please contact Customer Service within 30 days of receipt of purchase. Any returned item must be in new, unused and resalable condition and in the original packaging. Any shipping cost(s) associated with a returned product(s) will be at purchaser’s expense. Those products being returned that included free shipping will be subject to the outbound shipping costs, and deducted from any refund. All returned products will be inspected prior to any credit being issued.
How to Return a Product:
Step 1: Before a return can be processed you must first contact Customer Service at: (844) 719-2960 Monday through Friday, 8 a.m. to 5 p.m. Pacific Standard Time, and request a RMA number (Return Merchandise Authorization).
Step 2: Your original method of payment will be credited less restocking fees, if applicable, within 10 working days of receipt of the product(s) returned to MedRequisite.com in good, resalable condition. Any product returned in poor condition or determined not to be resalable may result in a denial of credit being issued.
Step 3: The costs incurred for returning a product(s) to the applicable warehouse address provided by MedRequsite.com with the RMA number, is the responsibility of the purchaser.
Step 4: If a defective product is received or shipped in error by MedRequisite.com, a credit, if originally charged, will be issued within 10 working days to the original form of payment. Shipping and restocking fees will not be applied.

This is end-to-end encryption that ensures your payment and personal information is safe. This type of information would include, but not be limited to, information provided at check-out or during the set-up of an “Auto-Ship Program” account.

Placing Orders or Participation in the “Auto-Ship Program”.
We request information from you on our Order Form. To buy from us, you must provide contact information (such as your name and shipping address) as well as your financial information (such as credit card number and expiration date). This information is used for billing purposes only and to process your order(s). If we have trouble processing your order, we will utilize this information to contact you.
We utilize an outside third party supplier to stock and ship orders, and a merchant services provider to process payment for products purchased. These companies do not retain, share, store or use personally identifiable information for any secondary purposes beyond filling your order and processing your payment.
